Kombucha and diabetes: labels, research and practical questions

Read the bottle, not a blood-sugar promise.

Kombucha is not a diabetes treatment, and fermentation does not make it sugar-free. Whether a particular drink fits your care plan is a question for your diabetes care team, using its actual nutrition information. We cannot recommend a dose, flavour or blood-glucose experiment from a brewery website.

Start with total carbohydrate information

Kombucha begins with sweetened tea. Fermentation uses sugar, but does not establish how much remains. Fruit ingredients can contribute additional sugars. A sour drink may still contain sugar; an unflavoured one is not automatically sugar-free.

The NIDDK’s diabetes guidance explains why carbohydrates, meal planning and individual treatment need to be considered together. It recommends working with your healthcare team rather than relying on a single “healthy” food.

When reading a bottle:

  • Find total carbohydrate and sugar information, not just a front-label claim.
  • Check whether values are per serving, per bottle or per 100 ml.
  • Compare products on the same volume basis.
  • Include juice, honey or syrup added when making a mixed drink.

What the human research does—and does not—show

A 2023 randomised crossover pilot study investigated one kombucha in adults with type 2 diabetes. Only seven participants completed the data needed for the fasting blood-glucose analysis.

The researchers reported a reduction from baseline during the kombucha period, but the overall comparison between kombucha and placebo at the end of treatment was not statistically significant. A smaller subgroup showed a different result. Attrition, self-reported readings and the use of a single study drink limit what can be concluded.

That is a reason for further research, not a prescription. The study did not test Scoby Doo, establish a suitable amount for you or justify changing medication. It also cannot be generalised to every type of diabetes.

Sugar is not the only consideration

Kombucha can contain caffeine and fermentation-produced alcohol. The US TTB’s kombucha guidance explains that alcohol can change with production and storage. Its legal thresholds apply in the US; they do not establish an Indian product’s composition or classification.

Make the conversation useful

Ask your care team whether an unpasteurised fermented drink fits your existing plan, what product information they need, and how your medicines or other conditions affect that decision. Continue the monitoring and treatment plan they have given you; do not replace it with a self-designed kombucha trial.
